NOTICE OF UDATING – CITY GARDEN – COVID-19 VACCINATION SCHEDULE FOR RESIDENTS

According to the latest from Ward 21 Health Station, Binh Thanh District regarding vaccination against Covid-19 for Residents have Residence certificate or temporary Residence, Management Office (MO) would like to update Residents as below:

Eligibility	Residents not yet vaccinated	Residents has 2nd vaccination due date Vaccine AstraZeneca: > 06 weeks Vaccine Pfizer: 03 weeks Vaccine Moderna: 04 weeks
Available vaccine	Temporary stop until further notice	Pfizer
Registration method		Register via link of Health Station: https://forms.gle/mjxGBYug9YLnnXZ5A Foreign Residents kindly refer to the attached photos below in English for registration.)
Time		Base on email or message sent by Authorities of Ward 21
Requirements		Having invitation email or message. Kindly bring your certificate of Permanent Residence or Temporary Registration and vaccination invitation.
NOTES Doctor will consider the suitable vaccine for 2nd dose. Vaccination location: Nguyen Trong Tuyen school – 294/22 Xo Viet Nghe Tinh, Ward 21, Binh Thanh District. People is being positive or used to be positive with Covid-19 do not receive the Covid-19 vaccine in 6 months after medical treatment. Vaccine type and quantity depend on actual allocation every day. Vaccination is on an individual voluntary basis.
