NOTICE – THAO DIEN WARD – CONTINUING TO ORGANIZE VACCINCATION FOR PEOPLE

Based on the Official Letter no. 482/TB-UBND from the Thao Dien Ward People Committee dated September 26, 2021 about continuing to organize vaccination for people in Thao Dien Ward, MO would like to inform to Residents as follows:

Date

From September 27, 2021 till further notice

Time:

Morning: 8:30AM – 11:30AM
Afternoon: 01:30PM – 05:00PM

The 2^nd dose of Pfizer vaccine
Eligible people	Residents from 50 years old who vaccinated 1^st dose of Astra Zenneca for 08 (eight) weeks Residents from 50 years old who vaccinated 1^st dose (Moderna or Pfizer) for 04 (four) weeks
Vaccination location	Venue 1: University of Culture, no. 51 Quoc Huong Street, Thao Dien Ward, Thu Duc City. Venue 2: Huynh Van Ngoi School, no. 78 Quoc Huong Street, Thao Dien Ward, Thu Duc City.
Method	Residents who had the inviting message via phone number that you registered as vaccinated 1st dose In case Resident is eligible for 2^nd dose but has not received the inviting message, please contact Mr. Than Trong Sang at phone number 0937 667 456 in the morning from 08:30 AM to 11:00 AM and afternoon from 02:00 PM to 04:00 PM for arrangement In case you have difficulty in contacting with the Ward staff, Residents please contact MO at hotline 0903 988 821 or the email Masteritd-CSM@savills.com.vn for timely assistance.

The 1^st and 2^nd dose of Vero Cell vaccine
Eligible people	People from 18 years old who haven’t vaccinated 1^st People from 18 years old who vaccinated the 1^st dose of Vero Cell for 03 (three) weeks
Vaccination location	Thao Dien Ward Medical station, no. 89nThao Dien Street, Thao Dien Ward, Thu Duc City.
Method	People please directly go to the vaccination venue and no need the inviting message

NOTE:

When Residents go to vaccination, please bring the health record (if any), your ID card/passport, pen, necessary personal thing (as drinking water, tissue, sanitizer…) and the 1^st dose vaccination record (if go for 2^nd dose)
After screening, in cases people are not eligible for vaccination, the Ward People’s Committee will compile a list to organize another vaccination at the health facility as further notification.
Residents please apply the disease prevention measures and 5K message when going to the vaccination.
